REQUIREMENTS FOR MINORS
PIERCINGS:
Age requirements:
8-11– Exclusively lobe piercings
12 and up- Any ear piercings
13 and up- Side nose piercing
14 and up- Belly button and Septum
16 and up- Eyebrow
*NO Dermal, Oral, Private or Surface Piercings on Minors*
All minor piercings require a ‘FL Department of Health Notarized Consent Form’ along with a valid form of state issued ID or passport for the parent/legal guardian.
If minor is under 15, a birth certificate OR a state-issued ID is required.
If minor is 15 and up, a form of ID (school ID, yearbook photo or Focus account) with birth certificate is required if ID is not state issued.
If last name of the minor and parent/guardian do not match we will require birth certificate or legal guardianship paperwork if applicable.

We DO NOT tattoo under the age of 16
All minor tattoos require a ‘FL Department of Health Notarized Consent Form’ along with a state issued ID for both child and parent/legal guardian. If last name of the minor and parent/guardian do not match we will require proof of name change or legal guardianship (divorce papers; marriage license; right of attorney; etc.)
